Application Form
Information on the application process is available under Admission & Enrollment. If you would like to apply for enrollment of your child, please follow these instructions:

1. Complete the "Application for Enrollment" PDF form* on your computer.

2. Print out the completed form

3. Attach a non-refundable check or money order payable to University at Buffalo Child Care Center. The application fee is $25 for student parents, $35 for all others.

4. Send the two items to us in person or via postal mail to either offices at:

6. We will call you when a spot becomes available.
